The FAA and Army have no system in place to ensure commands are heard in critical airspace near our nation’s capital. “This was preventable,” said NTSB Chair Jennifer Homendy. “The FAA knew this type of visual separation procedure in that airspace was dangerous. They just didn’t act fast enough.”
